To ensure your clinician is well prepared for your initial consultation, all clients are required to complete an intake form prior to their first appointment. You will receive a secure link via email once your appointment is scheduled.
The intake form includes:
The completion of this form is required prior to attending your appointment.
For diagnostic assessments, intake forms and questionnaires are sent approximately one month prior to your assessment date.
The questionnaires provided will vary depending on whether the assessment is for a child, adolescent, or adult. We use gold-standard, evidence-based, age-appropriate assessment tools.
All questionnaires must be completed at least one week prior to the assessment date. This allows your clinician sufficient time to carefully review and interpret the information before your appointment. If forms are not completed by this time, sessions will need to be rescheduled.

Session 1: Diagnostic Interview - in this 3-hour session, the psychologist will conduct a comprehensive diagnostic assessment. The focus will be on the assessment of Autism, however, other conditions and disorders will be considered. You will be interviewed about your life and asked about your social life, emotional regulation, work and school history, executive functioning, and childhood and developmental history. A variety of questionnaires will be provided within the session to complete.

Collateral information helps our clinicians develop a well-rounded understanding of the client and their day-to-day functioning.
For adult assessments, we ask that you nominate someone who knows you well (eg. parent, partner, relative, close friend, etc.) to complete questionnaires.
If you do not have someone you feel comfortable nominating, that is completely okay! Please notify Reception in your enquiry so we can let your clinician know.
For child assessments, we require input from your child’s teacher. Please notify the teacher that they will receive questionnaires via email, and that you will provide them a paper form after the parent interview.
For therapy appointments, payments are processed by Reception each day at approximately 11am using the card on file. An invoice receipt will be emailed once payment has been processed. Please ensure sufficient funds are available prior to 11am to allow for seamless processing.
For diagnostic assessments, a deposit is required at the time of booking to secure your assessment. The remaining balance will be processed one week prior to the assessment date. An invoice receipt will be emailed after each payment.
We understand that plans can change and unexpected circumstances arise. If you are unable to attend your appointment, please contact Reception as early as possible so we can offer the time to another client on our waitlist. Appointment reminders are sent via SMS 5 days prior and via email 3 days prior to your scheduled session. Cancellations made within 1 business day of the appointment will incur the full appointment fee. Cancellations made within 1-2 business days will incur 50% of the appointment fee.
For diagnostic assessments, a minimum of 5 business days’ notice is required to avoid the full assessment fee being charged, due to the significant preparation time involved.
Please note that cancellations must be communicated to Reception during our staffed hours (Monday to Saturday, 9am-5pm). If you contact us outside of these hours, your cancellation will be recorded as received on the next business day.
We appreciate your understanding that these policies help us maintain availability and provide consistent care to all clients.
Clinicians have prepared to deliver a quality of therapy within 50 minutes, with a 20-minute grace period. If you are 20+ minutes late to your appointment, the session will be considered a non-attendance, as the full therapy can no longer be provided in the remaining time.
If you have 3 or more cancellations within a 3 month period (without a medical certificate), then your clinician will be in touch to discuss phasing down or terminating the frequency of your appointment schedule.
Please know that we require all accounts to be settled before subsequent appointments will be confirmed.
If you have accessibility needs, communication preferences, or require adjustments at any stage of the booking or assessment process, please let us know. We are committed to providing an affirming and inclusive experience for all clients.
